Great guide and I admire your persistance and endurance to write such a long, and still interesting guide. However, there are some things that have to be pointed out:
1) Wind druids with enigma are easy, they will most likely teleport on you and try to spam, that's when smite pays off, however, if they choose for standing still and namelocking you with tornadoes.... hmm.... cyclone armor laughs at your FoH... hehe... Wind druids won't be very easy to win against.
2)FoH'ers... You seem to have dueled a lot of newbs here. My FoH'er, now deleted and traded had a 5sock caduceus, +3 conviction +3 FoH, with 5*5-5 Lightning facets. Perfect Griffon's with 5-5 Lightning facet and Exile. Conviction wasnt even near maxed, for the +skills giving it already the 150% caps. FoH dealing 8K+ damage and over 3K life + maxed block. Now the maxed block doesn't do anything good against your class, but ok. 1 point in fana, 1 in smite as the pre-req for Holy shield, maxed Holy shield, rest in defiance. Over 35K defence, which doesn't work against your class either, but ok:P. So, to start with I reduce your resist with 150%. No problem, your 4 Lo'ed shield, which gives +20% max lightning resist and some lightning resist (inherent resistances) maxed this out, combined with dual wisp, t-gods. I won't have a chance, but the same thing goes for you, so now we have to smite. Also you've stated smiters as noobs, while your build is simply Smiter and rest in FoH instead of Defiance/Charge. So now you have to use your n00b smite style against my n00b smite style. Hmm... what now... your charge won't hit me because I'm an Exile-n00b and you can't switch to angelic because you need dual wisp. Second I will use my ShaelUmTir (Crescent Moon) Phase blade. Because you need insane pre-buffing for my FoH your smite would be weak and I would still win.
3)Hammerdins own you, even if they DO teleport on you. I've never played a Hammerdin myself but I've dueled enough to know. They just spam hammerfields and have insane life. FoH = lol against them since the cast takes too long and does no dmg.
4) Zons can also throw their javelins, keep that in mind
