What I want to know is, why did they not catch that on the PTR? I mean, people were losing LOTS of money, I heard of one guy who lost a stack of primal mights.
The game was literally unplayable that day, because playing was risking losing your items. Stuff like that is what makes me question Blizzard's ability to maintain a game. I think Blizzard should just come up with the ideas, and let someone else actually run the game.

Anything on the PTR generally makes it onto Live, unless it was a beneficial change. It sounds cynical, but there are literally tons of game-breaking bugs like the one you listed that make it from PTR to Live, it's pretty hilarious.
I think that was the patch on PTR where they were testing the idea of Devastate attacking with both weapons a la Stormstrike, which was removed for doing "too much damage". I was hopeful too, it would have meant I could actually play my Warrior outside of a Raid/Instance.
