It's best to just let him come up and keep hitting you, since his jab attacks suck, and his charge attack hurts.

He can smite and then charge, though.

I usually stand against a wall so the smite attack won't push me back.

Ididn't die on Duriel but i was close to do that several times. I think it's the hardest monster to beat at the level u have.

even in .9 duri in hell seems to be harder then baal... i duno why...

Hell Duriel was impossible for me in HC back when I used Dial-up...

However did anyone turned into a Mephisto when using Delerium?
So far i have only been transformed into a Stygian Doll.

Does this answer your question. Maybe he just meant delerium and said breath.

The question is, could you actually turn into Mephisto? All the others have only turned into Stygian Dolls.
